The objective of MDS coordinators is to promote the physical andemotional well-being of CareMeridian residents. They use a Resident AssessmentInstrument (RAI) to gather information from residents and their families duringinitial and periodic interviews. The MDS covers such areas as the patient'smood, behavior patterns, cognitive ability, and nutrition needs.
Information from these assessments helps our interdisciplinaryteam to formulate individualized care plans that include support fromsocial services, dieticians, rehab specialists, and medical staff. MDScoordinators then implement and monitor these care plans to ensure theireffectiveness. They must also make sure all strategies comply with Medicarerequirements and ethical standards.
- Graduate of an accredited Registered Nursing (RN) program
- One year of related experience
- Experience working with medically complex individuals
- Current RN license
- Other education and experience plus licensure as required by state
